There is a moment in most Indian families when someone starts writing things down. A name that used to arrive instantly now takes a beat. It is one of the quieter anxieties of getting older, and it is the reason a small line of research into saffron has drawn attention far outside the spice trade.
What the trials found
The first serious test came from Akhondzadeh and colleagues in Tehran in 2010, published in the Journal of Clinical Pharmacy and Therapeutics. Forty-six people with mild-to-moderate Alzheimer's disease took either 30 mg of saffron a day or a placebo for sixteen weeks. Those on saffron scored better on the ADAS-cog, the standard cognitive scale used in dementia research. Adverse events were no more common than on placebo.
Several trials followed, some comparing saffron directly against donepezil and memantine — the drugs actually prescribed for Alzheimer's. In 2020, Ayati and colleagues pooled the randomised trials in BMC Complementary Medicine and Therapies and found saffron improved cognitive scores against placebo, and was not significantly different from the conventional medicines. A separate systematic review the same year by Avgerinos and colleagues, covering five studies and 325 people, reached the same conclusion and added the caveat that matters: the studies carry a meaningful risk of bias, and the promising results should be read cautiously.
The result that complicates the picture
We want to put this one in front of you rather than leave it out. In 2022, Marzabadi and colleagues gave sixty people with mild-to-moderate Alzheimer's donepezil plus either saffron or placebo for twelve weeks. On cognition, saffron added nothing — the difference in MMSE scores was not significant. What it did change were the blood markers: inflammation and oxidative stress fell, and total antioxidant capacity rose.
That is a genuinely useful finding, and it points at the honest state of the evidence. Saffron appears to be doing something measurable in the body. Whether that reliably translates into better memory, on top of existing treatment, is not settled.
Why a spice would do anything at all
The mechanism proposed across this literature is not mystical. Crocin and safranal — the colour and the aroma — behave as antioxidants and appear to interfere with the aggregation of amyloid and tau, the proteins that accumulate in Alzheimer's. Laboratory and animal work supports this consistently. Human work is where it thins out.
What this means at your kitchen table
The trials used around 30 mg a day as a supplement, roughly a generous pinch of threads, taken under medical supervision in people already diagnosed. That is a different thing from cooking with saffron, and neither is a substitute for a neurologist. If memory is changing in someone you love, that is a conversation for a doctor, not a spice jar.
What you can take from it is gentler and, we think, nicer: the daily pinch of kesar that has been in Indian kitchens for centuries is not an empty ritual. It carries compounds that serious researchers are studying seriously. The evidence is early. The tradition is not.
