Saffron turns up constantly in writing about heart health, usually with the word "powerful" nearby. The literature is more interesting than that, and quite a lot more modest.
What the meta-analyses found
In 2021, Setayesh and colleagues pooled eight randomised trials in Nutrients. Saffron supplementation lowered systolic blood pressure by about 0.65 mmHg and diastolic by about 1.23 mmHg. Both were statistically significant. The authors then added a sentence that most wellness articles quietly drop: the effects were small and may not reach clinical importance.
Pourmasoumi and colleagues, pooling ten studies in Pharmacological Research in 2019, found much the same — diastolic pressure down about 1.24 mmHg, modest weight and waist reductions — and no significant change in the lipid profile at all.
A larger 2022 dose-response analysis by Zamani's group, covering thirty-two studies and 1,674 people, was more favourable on lipids: triglycerides down about 8.8 mg/dL, total cholesterol about 6.9 mg/dL, LDL about 6.7 mg/dL, alongside falls in inflammatory markers and a rise in total antioxidant capacity.
The review worth reading twice
In 2021, Kadoglou and colleagues published a deliberately critical appraisal in Phytotherapy Research. Their verdict on the clinical trials: mostly weak effects, a negligible hypotensive effect, controversial results on lipids, and important design problems throughout — small numbers, short durations, wildly varying doses, no pharmacokinetic data.
They were not dismissing saffron. The laboratory and animal evidence for its effects on the blood vessels is substantial, including atherosclerosis regression in animal models. Their point was that the human trials have not yet been done well enough or long enough to make claims from.
Why the gap between the lab and the clinic
It is worth understanding why those two bodies of evidence disagree, because the pattern repeats across nutrition research. In a dish or a mouse, you can give a large, precisely measured dose of a purified compound directly to the tissue you care about, for as long as you like. In a human trial you give a modest dose of a whole spice, by mouth, for eight or twelve weeks, to a few dozen people whose blood pressure is already being managed by medication, diet and everything else in their lives. The compound has to survive digestion, reach the bloodstream in useful quantity, and then produce a change large enough to see through all that noise.
A trial like that is well designed to miss a small real effect. It is also well designed to produce an exciting result by chance. Which is exactly why the meta-analyses matter more than any single study, and why the sensible thing to watch for is a long trial with hard endpoints — heart attacks and strokes, not millimetres — that nobody has yet funded.
So what is a fair thing to say
That saffron nudges several cardiovascular measures in a favourable direction, by a small amount, fairly consistently. That the effect on cholesterol is genuinely disputed between analyses. And that a millimetre of mercury is not a blood pressure treatment — walking, salt, sleep and prescribed medication are, by orders of magnitude.
We would rather tell you that than repeat "powerful antioxidant" and hope you don't check.
